Job description

Are you a dedicated Nurse Assessor looking for exciting new opportunity? Gap healthcare are currently recruiting for Senior Nurse Assessors to support our client based in Birmingham.

You will play a critical role in the assessment and review of patients in line with CHC. This involves both patient-facing and remote working components, ensuring comprehensive and holistic assessments are conducted effectively and efficiently.

Location – Birmingham/Hybrid

Hours – Monday to Friday (37.5 hours)

Rate of Pay – £40,000 to £45,000 PA

Key responsibilities:

  • Conducting detailed assessments using the Decision Support Tool DST for CHC eligibility, ensuring compliance with the National Framework.
  • Managing a caseload of patients requiring CHC assessments, reviews, and case management.
  • Completing at least four DSTs per week.
  • Undertaking face-to-face assessments across Birmingham and Solihull.
  • Liaising with multidisciplinary teams, patients, and their families to gather necessary information and provide clear communication throughout the assessment process.
  • Attending and contributing to multidisciplinary team meetings and panels.
  • Keeping up to date with current CHC legislation, policies, and guidelines.

Requirements:

  • Registered Nurse (valid NMC registration).
  • Significant experience in Continuing Healthcare (CHC) assessment and case management
  • Strong communication and interpersonal skills.
  • Ability to work autonomously and manage a diverse caseload.
  • Proficiency in using electronic patient record systems and Microsoft Office Suite.
  • Valid driver’s license and willingness to travel for patient assessments within the Birmingham and Solihull area.
